v5 patch: full VoiceOver support + Zoom cursor tracking
Added full NSAccessibility text protocol for VoiceOver: - accessibilityValue (buffer text, capped at 10k chars) - accessibilitySelectedText (active region) - accessibilitySelectedTextRange (cursor position) - accessibilityInsertionPointLineNumber - accessibilityVisibleCharacterRange - accessibilityStringForRange: - accessibilityAttributeValue: delegating to modern methods Plus legacy parameterized attributes (StringForRange, BoundsForRange). UAZoomChangeFocus retained for Zoom 'Follow keyboard focus'.
