v4 patch: dual UAZoomChangeFocus + NSAccessibility, with Apple doc refs

UAZoomChangeFocus is Apple's documented API for Zoom focus control.
NSAccessibility serves VoiceOver and other AT tools.
Both are needed - same approach as iTerm2.
